MA Forms Adminstration (District/Super Administrators Only)

1. MA Administrative Setup

In order to use MA Forms, you must have the optional MA Forms module installed for your district.

1.1 Navigate to the MA Reports Menu

  1. From the Administrative Menu click on Reports.
  2. In the MA Forms section (toward the bottom of "Built-in reports") click on "MA Forms reports" to open the MA Reports Menu.

NOTE: District Administrators will go directly to their district's MA Reports Menu. Super Administrators will have the opportunity to select/change the district they want to work with.

1.2 Initial Setup

All of the District/Provider Information on the District Setup screen must be completed before batch files can be uploaded to MN-ITS. MA Services and their billing rates must be completed before IEP Managers can create Activity/Trip Logs.

1.2.1 District Setup
  1. Navigate to the MA Reports menu.
  2. Click on the "District Setup" link.
  3. In the District Billing Setup screen, follow the "Format" examples (if noted) to fill in all fields that are listed (all must be filled in for X12 Compliant Batch files to be accepted).
  4. NOTE: The zip code must be the nine digit zip code and cannot contain any hyphens or dashes. The billing phone number also cannot contain any spaces or special characters. These are requirements for X12 Compliancy.

1.2.2 Services Setup
  1. Navigate to the MA reports menu.
  2. Click on the "Services Setup" link.
  3. Make changes to or add services as needed.
  4. NOTE: If both billing rates are left blank, that service will not be available on Activity/Trip Logs.

    NOTE: When using the "Effective date" the "Previous billing rate" will be calculated for services prior to this date and the "Current billing rate" will be used for all services falling on or after the "Effective date".

1.2.2.1 Updating Billing Rates to the New Rate
  1. Navigate to the MA reports menu.
  2. Click on the "Services Setup" link.
  3. Copy the rate in the "Current billing rate" column to the "Previous billing rate" column.
  4. Fill in the new rate in the "Current billing rate" column.
  5. Make sure the "Effective date" is set to reflect the date that the new rate will take effect.
  6. NOTE: The "Previous billing rate" will be calculated for services prior to this date and the "Current billing rate" will be used for all services falling on or after the "Effective date".
