9/17/07: New, improved, alternate IEP services page
SpEd Forms has developed an alternative to the MDE recommended Services Delivery page. We have attempted to address all the new documentation requirements while putting them into a more user-friendly format.
The SpEd Forms Alternative Services Delivery page contains the following features:
It adds back the Special Education and Related Services grid, containing both direct and indirect service time, from the prior IEP.
It creates a separate data field for Adaptive Equipment, with “No” and “Yes, explain” check boxes, followed by an expanding text box where the adaptive equipment can be described.
It creates a separate data field for Special Transportation, with “No” and “Yes, explain” check boxes, followed by an expanding text box where the special transportation can be described. (This feature also allows administrators to create a list of all students within a building, district or cooperative who require special transportation.)
It creates a separate data field for Paraprofessional Support, with a “No” and “Yes” check boxes, with the option to add one or more services grids for paraprofessional support, each followed by an expanding text box to describe the specific duties of that paraprofessional. An administrator definable pull-down menu has been created to select the various types of paraprofessional support.
It adds back the “Yes, No, More data needed” check boxes for ESY services from the prior IEP, adding the option to add one or more ESY service grids when the student qualifies for ESY.
Adds both a pull-down menu and an expanding text box for “Program Modification, Supports and Adaptations.” (SpEd Forms is developing a master list of modifications, supports and adaptations that will be available later this fall within this pull-down menu.) Note: This area is designed so that each modification, support or adaptation is entered into a separate data field, with the option to add as many data lines as needed.
The LRE explanation is entered into an expanding text box.
The alternative page can be accessed via the Preferences option in the Super Administrator main menu, selecting the “SpEd Forms Customization” option on the next screen, checking the “Use alternate service delivery page.” under the Service Plans heading, and then saving that choice.
