5/8/08 Recent improvements

Please find a summary of improvements to SpEd Forms following. The improvements have been posted to all SpEd Forms servers.

Administrators:

1. A guide to creating custom reports is now linked to from the word "Help" at the top of the custom reports menu.

IEP:

1. In the "Record or Status of Proficiency in meeting Diploma Requirements" grid on page 6.2 of the IEP, entry fields have been added for GRAD/BST - Writing.

2. A new rule has been added to the validation script. Service dates must now start on or after the IEP Team Meeting date.

504:

1. The "504 Accommodation Plan" has been updated.

2. "504 Parent Rights" added.

3. "504 Complaint Procedure" added.

4. "504 Parent Permission" added.

5. "504 Eligibility Determination" added.

ECSE:

1. "Child Outcomes Summary Form" added.

2. Specific "Prior Written Notice" added.

3. Part B and Part C options added to the "Parental Consent/Objection Form".

Notice of Educational Evaluation/Reevaluation Plan:

1. The wording above the second expanding text box has been changed from "Following is a statement of adaptations needed to conduct this evaluation:" to "Statement of adaptations needed to conduct this evaluation:".

2. An additional expanding text box has been added titled "Describe the other options considered and the reasons why those options were rejected".

MA Forms:

1. PDFs of the MA Parental Consent form have been added.

2. NPI now sent with each batch transmission.

3. Navigation issues in the "Create/Edit Batch Files" screen have been fixed.

4. A search criteria for unpaid claims has been added to the "Create/Edit Batch Files" screen.

5. "Date of Service" sort on the "Create/Edit Batch Files" screen now uses student last name and first name as secondary sort criteria.

Summary of Performance:

1. The "Post Secondary Goals" section now lists the post secondary goals from the "Transition Planning" rather than transition goals.

Authorization for Release of Information:

1. Staff person responsible and district/school address added for clarity next to "Authorizes".

General:

1. To reduce confusion with "one to one" paraprofessionals, all instances of "Individualized Paraprofessional Support" have been changed to "Child Specific Paraprofessional Support".

2. For consistency, instances of "Case Manager" have been changed to "IEP Manager".

3. Additional contact information added to teacher profiles.

4. MARSS Status End code "09" has been removed.
