Prior Written Notice, Parental Consent and Notice of Evaluation

The following changes have been posted to the Prior Written Notice, Parental Consent/Objection and Notice of Educational Evaluation/Reevaluation.

Prior Written Notice:

1. A new text area titled "Description of other factors affecting the proposal or refusal" has been added.

2. Predefined drop down menus have been added to all text areas. These drop down menus may be modified in administrative preferences.

3. Removed "This form pertains to the Notice of Educational Evaluation/Reevaluation dated:".

4. The page break was moved from right above "When a district proposes to conduct an evaluation…" to right above "Here are some agencies that may help you…".

Parental Consent/Objection Form:

1. Removed "This form pertains to the Notice of Educational Evaluation/Reevaluation dated:".

2. The Parental Consent/Objection Form now prints automatically with the "Notice of Educational Evaluation/Reevaluation".

3. The last line of the "Parental Consent/Objection" form will no longer print on a second page.

Notice of Educational Evaluation/Reevaluation (08/09 version only):

1. Added "Description of each evaluation procedure...".

2. Added "Other factors affecting this proposal...".

3. Predefined drop down menus have been added to all text areas except "Statement of adaptations...". These drop down menus may be modified in administrative preferences.

4. The form will now be printed on one page instead of two.
