2008/09 IEP Posted
The SpEd Forms 08/09 IEP has been posted. The IEP is based on the new state recommended IEP released in November with modifications recommended by the SpEd Forms forms committee.
Following is an overview of differences between the SpEd Forms 07/08 IEP and the SpEd Forms 08/09 IEP.
Key:
[S] = Reflects changes to the state recommended IEP.
[C] = Recommended by the SpEd Forms forms committee.
[P] = Programming comment.
Page 1 - Student Information:
1. [S] The "Indicate Attendance" check boxes have been removed from the list of IEP team members.
2. [C] Under "Type of IEP" the "Initial" has been changed to "Initial placement".
3. [C] An administrative preference has been added to allow removal of the "Interim" check box.
Page 2 - Student Performance:
1. [S] Moved to page 2.
2. [S] Removed "How will progress toward meeting this annual goal be measured?".
Page 3 - Transition Services (Page 1):
1. [S] Moved to Page 3.
2. [S] Renamed from "Transition Planning" to "Transition Services".
3. In "Courses of Study":
a. [S] Removed default grade levels.
b. [S] Removed "Credits Earned".
c. [S] Removed "Total number of credits required"
4. [S] "Measurable post-secondary goals" categories have been reduced from five categories to three.
5. [P] For convenience of teachers post secondary goals previously entered under the eliminated categories ("Home Living" and "Community Participation") are auto-filled into "Independent Living".
Page 4 - Transition Services (Page 2):
1. [S] Transition service needs section was substantially changed. The old areas were replaced with five new areas.
2. [P] Since the new transition services needs areas do not match the old areas information could not be automatically transferred from the 07/08 IEP.
3. [C] The last sentence under "Transfer of Rights at Age of Majority" was changed to "(Student first name) was informed on ________ (date) that the rights will transfer on their eighteenth birthday." The words "informed on" link to relevant instructions from the IEP guide.
Page 5 - Services:
1. [C] "Child Specific Paraprofessional Support" was moved directly below "Special Education and Related Services".
2. [S] The language under "Least Restrictive Environment" was changed.
3. [C] "Program Supports for School Personnel" was added under "Program Modifications, Supports and Adaptations in General and Special Education".
Page 6 - State assessments:
1. [S] Under "State Assessments for accountability" "(Effective 2007-08)" was removed.
2. [S] Under "Diploma Requirements" "Pass State" was changed to "Pass State for BST; Pass for GRAD".
3. [P] Drop down menus have been added for assessment accommodations.
Progress Report:
[C] To report progress for the 08/09 IEP, teachers must select a finalized IEP. Progress reports now include cumulative progress.
[P] Information from old progress reports does not automatically transfer from the 07/08 Progress Report to the 08/09 Progress Report due to potential issues with goal reordering and changes when the IEP is revised.
