SpEd Forms: Preview of improvements (MN)
Following is a preview of changes to be posted to SpEd Forms servers on Sunday 03/14/10.
Highlights:
1. Comprehensive new Autism Spectrum Disorder goal library.
2. New Prior Written Notice, Notice of Evaluation and Parental Consent/Objection forms for ECSE.
Administrators:
1. A new preference has been added: "Agreement to Amend: replace signature lines with documentation of parent contact".
2. A new preference has been added: "Transition: Show credits required for graduation."
3. A "Grades" section was added to the school setup to conform with MARSS A reporting requirements.
4. The service start date (SSD) column on the "Combined Report" now reflects the earliest related services start date.
MA Forms:
1. The "AT Claims" feature has been updated to accommodate the "Attachment Control Number" in batch transactions.
2. A "View other insurance" has been added to the "MA Billing Setup" page if the student has other insurance information available.
3. A "sent" column has been added to the "MA services on no-school days" custom report. To take advantage of this update your super administrator must delete the original report and then run the "Update Database" script in "SpEd Forms maintenance".
4. The "Import eligibility response (271)" process has been updated so that students from other districts will not be included on the print page after importing.
5. When the "MA Parental Consent" is printed all pages will now be printed together and, in most cases, each page should no longer be split into two pages.
6. The form that was previously called the "Physician Statement of Need" is now entitled the "Request for medical orders".
7. A new line for "Related to the following conditions/diagnosis" has been added to the "Request for medical orders" that will only appear on the print page if it is NOT filled in.
8. The print page has been updated on the "Activity Log" so that the total number of "Encounters" would be calculated correctly.
9. The electronic signature feature has been added to the supervisor's name of the "Trip Log". Once the supervisor has "signed" the log, users will no longer be able to add or edit trip entries on that log.
10. The trip log has been updated to conform with new DHS requirements.
General Ed (MARSS, Interventions, 504 Plan and ESL):
1. The tabs for "504 Plan" and "ESL" have been removed and these menus now appear in the "General Ed" tab.
ER Forms:
1. "Miller Function and Participation Scales (M-FUN)" has been added to the "Motor" section of SpEd Forms Test Templates.
Service Plans:
1. Progress report: A "Show objectives" link has been added which automatically adds objectives to the progress comments.
2. Progress report: The following statement was added: "The extent to which that progress is sufficient to enable the pupil to achieve the goals by the end of the year".
IEP (2010)
1. Goals: A drop down list has been added for "How will progress toward meeting this annual goal be measured?" (PROGRESS MEASURES (88)).
2. Goals: An extensive goal library for "Autism Spectrum Disorder" has been added.
3. Transition 1: "Grade 8" has been removed from the "Grade Level" under "Courses of Study".
4. Transition 2: The statement "The date [student] and parent was informed..." has been changed to "The date [student] and parent were informed...".
5. Services: The drop down that was directly under "Program Supports for School Personnel" has been removed. The choices that were in that drop down were added to the "Service" drop down for that service.
6. Services: Optional service sub-menus have been added to "Special Education" and "Extended School Year" services.
7. Services: Validation now checks to make sure that there is at least one support included under "Program Supports for School Personnel".
8. Assessments: The drop down list for accommodations (TESTING ACCOMMODATIONS (55)) has been updated.
IFSP (2009/2010)
1. A "Progress Report" has been added to the "IFSP" (this only appears on the 2009 and 2010 form collections).
2. The "method" drop down on the services page has been updated.
Early Childhood Special Education
1. Prior Written Notice, Notice of Evaluation and Parental Consent/Objection forms were replaced with:
